The Cultist Priest leads small warbands of the Tarkov cult, and everything about the fight breaks the rules the other bosses follow. He only appears at night, he does not announce himself, and his preferred opening move is a knife in your back before you knew anyone was there.
The knives are poisoned. Survive the initial stabbing and you are on a timer, taking damage over time that can kill you long after the cultists are gone if you cannot treat it.
Cultists will also use guns when pressed, but their identity is stealth. On night raids across their maps, the scariest sound in Tarkov is no sound at all.
